Lemierre’s syndrome: Presentation of Two Cases and Review of the Literature

Summary

The authors present two clinical cases of Lemierre's syndrome. The first patient is a 30 year-old woman and the second a 75-year-old man. Both presented clinical features suggesting a laterocervical phlegmon or abscess. The symptoms followed an acute tonsillitis episode resistant to empirical antibiotic therapy. The computerized tomography scan exhibited a thrombosis of the left internal jugular vein extending from the skull base to the brachiocephalic vein in both cases, with involvement of the sigmoid sinus in the second case. The first case had secondary involvement of lungs and heart. Anticoagulation was used in both cases and despite its use, they remain with persistent thrombotic occlusion of the left internal jugular vein.

With these cases we point out several Lemierre's syndrome particularities as the diagnosis challenge, complications, antibiotherapy and use of anticoagulation in the management of this rare disorder.
